To better serve our patients and enhance your experience, we will be performing important system upgrades on Tuesday, June 23rd, and Wednesday, June 24th.

AS A RESULT:

• Urgent Care will close early at 5:00 PM

Wednesday, June 24, 2026
• Urgent Care will open at 10:30 AM (30-minute delayed opening)

These upgrades will help us improve our systems and continue providing the highest quality care to our patients.

📢 Important Notice: New Proxy Consent Requirement for Minors

To ensure the safety, privacy, and proper authorization of medical care for our patients, Jacksonville Children’s Multispecialty Clinics (JCMC) has implemented a new Proxy Consent Policy.

Effective immediately, any minor presenting for an appointment without a legal parent or legal guardian must have a completed Proxy Consent Form on file authorizing another adult to accompany the child and consent to treatment.

Parents and legal guardians are encouraged to visit any JCMC location to complete a new Proxy Consent Form and designate approved caregivers, relatives, or other responsible adults who may bring their child to appointments.

Failure to have a completed Proxy Consent Form on file may result in delays or the inability to provide treatment until proper authorization is obtained.

Dear Valued Patient,

With heartfelt gratitude, we would like to share an important update regarding our JCMC Allergy, Asthma and Sinus clinic.

We are deeply saddened to inform you that Michelle Synder, NP, will be departing from our practice.

Michelle has been a dedicated and compassionate provider, and we are grateful for the care she has provided to so many of our patients and families. Her commitment to patient care has made a meaningful impact on our clinic and community.

To ensure continuity of care, we are pleased to share that Dr. Guillot will continue providing oversight of your care, with Kim Lewis, PA, serving alongside him to support patients and help ensure there is no interruption to your healthcare needs.
