Care Management Team:

Who we are:

Care Management is a multidisciplinary team of nurses, social workers, medical assistants, community health workers, and care coordinators who work together to help meet patient health goals. We work in tandem with your medical team including your primary care providers and specialists at Jacksonville Children’s Multispecialty Clinic. We also work within the community to bridge care gaps, collaborate with governmental entities to serve our patients, and advocate with specialty providers at other healthcare locations to ensure management of complex health conditions. We provide holistic care to patients by focusing on assessment of the social determinants of health factors that affect our patients ranging from financial hardship, health literacy, healthcare access, food insecurity, housing, and social support. We approach all our patients from a patient centered lens. Every member of our team ensures that patients collaborate in their health goals thereby increasing their ability to successfully navigate the healthcare landscape more efficiently and effectively. We are passionate advocates, cheerleaders, and innovative problem solvers here to help the community at large. 

Who we serve:

Care Management serves individuals and families that receive Managed Medicaid and see JCMC for primary care services. Our care is focused on those who: ​

  • Have unmanaged complex medical or mental health conditions ​
  • Have unmet needs in the areas that most affect our health and quality of life, or the social determinants of health: Food, Housing, Financial Stability, Childhood Development, Health Literacy, and Safety. ​
  • Require additional support in navigating the healthcare system to reduce incidence of chronic ER usage and increase a patient’s ability to access appropriate and equitable care ​
  • Need assistance in leveraging their Managed Medicaid Healthcare Benefits
