Dear Valued Patient,

With heartfelt gratitude, we would like to share an important update regarding our JCMC Sneads Ferry clinic.

We are deeply saddened to inform you that Jennifer Schofield, NP, will be departing from our practice next month as she pursues a new career in another specialty.

Jennifer has been a dedicated and compassionate provider, and we are grateful for the care she has provided to so many of our patients and families. Her commitment to patient care has made a meaningful impact on our clinic and community.

To ensure continuity of care, we are pleased to share that we have secured a new provider who is scheduled to join our practice in August. In the interim, we are in the process of securing a temporary physician to support Jennifer’s patients and help ensure that there is no disruption to your care needs. We anticipate this provider beginning in April (start date to be determined).

Our team is committed to making this transition as smooth as possible. If you need to schedule an appointment or have any questions regarding your care, please do not hesitate to contact our office, and our staff will be happy to assist you.

Jennifer Schofield, NP, is a dedicated healthcare professional with a passion for providing personalized care to patients of all ages. With a career spanning over a decade, Jennifer has acquired a wealth of experience and expertise in nursing and family medicine. Her journey in the field of healthcare is marked by continuous learning and a commitment to patient well-being.

Jennifer’s educational journey began in 2012 when she earned her Associates Degree in Nursing. Her initial years were spent in the fast-paced environment of the Emergency Room, where she honed her skills and gained valuable experience in critical care.

Recognizing the importance of advanced education in healthcare, Jennifer returned to school in 2018 to pursue a Bachelor of Science in Nursing (BSN) and furthered her studies with a Master of Science in Nursing-Family Nurse Practitioner (MSN-FNP) in 2019. Her dedication to professional growth led her to become board certified through the American Association of Nurse Practitioners (AANP).

Jennifer’s career has taken her across state lines, from Milwaukee, Wisconsin, to North Carolina, where she now resides. The move was prompted by her desire to be closer to family, reflecting her strong commitment to both her professional and personal life.

Jennifer is not only a dedicated healthcare provider but also a loving spouse and a mother of two daughters. She cherishes the quality time spent with her family, frequently taking advantage of their new coastal surroundings to enjoy the beach and create lasting memories together.

In her role as a Nurse Practitioner, Jennifer finds immense satisfaction in helping patients achieve their personal health goals. She believes that every individual is unique and, therefore, deserves a customized care plan tailored to their specific needs. Jennifer’s empathetic and compassionate approach to patient care sets her apart, making her a trusted partner in her patients’ health journeys.

Jennifer embraces the diversity of her work, caring for both adults and pediatrics.
